MIDDLETON-Hazel I. Kampschoer, age 88, passed away on Sunday, December 2, 2012 at Oak Park Place . She was born on May 30, 1924 in Rudolf, WI the daughter of Anthony and Anna (Joosten) Johnson. Hazel was united in marriage to Charles Kampschoer on May 24, 1947. He preceded her in death in 2007. She worked for the Middleton-Cross Plains School District. Hazel was a member of St. Bernard’s Catholic Church and was active in the church Rosary group.

She is survived by three children, Charles (Lori) Kampschoer, Paula (Thomas) Blankenheim and John (Margaret) Kampschoer; six grandchildren, Heather, Katie, Jeff, Jenny, Adam and Dana; her siblings, Earl Johnson, Marie Hamm, Lois Bathke, Armelda Koller, Jane Johnson and Jerry (Darlene) Johnson and many nieces, nephews and other relatives and friends. In addition to her husband, Charles Kampschoer, Hazel was preceded in death by her parents, two brothers, Harry Johnson, Clayton Johnson and a sister, Mildred Steege.

A Mass of Christian Burial will be held at ST. BERNARD’S CATHOLIC CHURCH, 7450 University Ave., Middleton at Noon on Friday, December 7, 2012 with Monsignor Douglas Dushack presiding. Entombment will be in Resurrection Cemetery, Madison. Visitation will be held on Friday from10:00 AM until the time of the service at the church. In lieu of flowers, memorials may be made to the Alzheimer’s and Dementia Alliance, 517 N. Segoe Rd., Madison, WI 53705 or to the National Kidney Foundation, 30 East 33rd Street, New York, NY 10016.Gunderson_Funeral

The family wishes to extend a special thank you to Oak Park Place and SouthernCare Hospice for the loving care they gave Hazel.

Memorials:
In lieu of flowers, memorials may be made to the Alzheimer’s and Dementia Alliance, 517 N. Segoe Rd., Madison, WI 53705 or to the National Kidney Foundation, 30 East 33rd Street, New York, NY 10016.

Visitation:
Visitation will be held on Friday from10:00 AM until the time of the service at the church.

Service:
